Vacancy: Case Manager Purpose of Post: This is a 12-month fixed-term contract and the post is based in Gabriel Lodge, Ahiohill, Enniskeane, Co Cork (formerly Cara Lodge). Some of the key duties of the role include: Provide a welcome and supportive environment for all residents. Complete comprehensive assessments and develop an appropriate Care Plan based on identified needs in areas of: nutrition, physical and mental health, life skills, social skills, education / work, addiction support, counselling, accommodation, etc. Build a relationship with residents based on trust & respect. Encourage and support the person in developing and implementing an agreed holistic support plan, including day and activity programmes. Ensure a programme of social, recreational, educational and work activities is organised by or with residents as appropriate. The ideal candidate for the role will have: High level of Communication and interpersonal Skills. 2 years of experience in a same or similar role. Experience of case management and key working. Previous experience of voluntary / statutory agency working with marginalised groups, e.g. homeless people, prisoners, ex-prisoners, care leavers, travellers. Experience of working with people with addictions including those with mental health issues / dual diagnosis. Strong communication, interpersonal and listening skills. Knowledge of: homelessness, housing assistance structures, social deprivation, advice and information provision, etc. Very strong organisational skills, time management, work planning. The ability to: work under pressure, as part of team, keep statistics. Non-judgemental understanding of addiction. Relevant Third Level Degree, e.g. Social Care / Social Science /Youth & Community.
