Kerry) - Permanent Contract Purpose of this post: Support people who are sleeping rough and/or experiencing long-term and multiple exclusion homelessness to move into secure, permanent housing at the earliest opportunity, without preconditions for sobriety or treatment compliance. Support people prior to, during, and following their move through Key Working and Case Management to build their capacity to meet their goals, address their support needs, and sustain their housing. Some of the key duties of the role include: Identify and engage with individuals experiencing homelessness in County Kerry. Assess housing options and facilitate access to suitable accommodation. Develop and implement support plans tailored to individual needs. Provide intensive case management and ongoing support to tenants. Advocate for tenants' rights and address any issues that may threaten housing stability. Assist with induction to accommodation and familiarize tenants with their rights and responsibilities. Manage finances, including addressing rent arrears and ensuring access to benefits. Maintain accurate records and generate required statistics for reporting purposes. Collaborate with team members and external agencies to achieve project objectives. Adhere to health and safety regulations and participate in relevant training. Report to and seek guidance from the Team Leader of the South West Region Housing First. The ideal candidate for this post will have: Two years’ experience in a similar role. Experience of case management and key working. Experience working with people with addictions including those with mental health issues / dual diagnosis. Third level degree in a relevant field. Non-judgemental understanding of addiction. Belief in the potential of recovery for people of diverse backgrounds, challenges and personal circumstances. Support Team Leaders and Project Workers to implement care plans and promotion of independent living skills aimed at motivating people to move on to more appropriate accommodation. Some of the key duties of the role include: On first contact identify immediate need and refer to other services where appropriate. Provide bed space in appropriate accommodation based on initial assessment / referral from other projects. Carry out Health and Safety checks and follow relevant procedures. Provide for personal hygiene / showers, clean clothes, laundry, meals, clean rooms to a high standard. Assist in implementing a programme of social activities as organised by or with residents and people using services as appropriate. Assist in implementing a programme of social activities as organised by or with residents and people using services as appropriate. Act as an advocate when appropriate. Build a relationship with residents based on trust & respect by accepting people as they are, spending time with them, actively listening and challenging them positively and proactively to make positive changes at a pace suitable to each person. For more information about the role please download our Recruitment Pack: Download Recruitment Pack Download Application Form Outreach Project Worker - Fixed-Term Contract Purpose of this post: Seek out and provide information, assistance, assessment, support and referral for people who are homeless, at risk of homelessness, sleeping rough or in unstable accommodation and have issues with alcohol, drugs or substances. Act as a bridge to Cork Simon and other services Some of the key duties of the role include: Spend time on the streets, on Soup Run, in the Day Service/Rough Sleeper Service and in the Emergency Shelter as required to enable contact. Proactively look for people who need Cork Simon and other services Advocate for and support people in accessing accommodation, medical services, addiction services, rights and entitlements. Carry out detailed assessments of need to inform effective care planning. Develop care plans with people in co-operation with the Day Service/ Emergency Shelter staff and other relevant medical and mental health services. Develop harm reduction programmes and supports for those who are not ready, willing or able to abstain from using alcohol/drugs/substances. Maintain accurate and up to date records/ service user files to facilitate provision of care. Liaise and work with external agencies as appropriate. Provide supervision to relevant staff and/or volunteers/students. The ideal candidate for this post will have: Experience Working with addictions (Drug & Alcohol users) and mental health issues. 2 years previous experience in care setting. Have knowledge of homelessness and social deprivation. Experience supervising staff/volunteers. Relevant Third Level Degree e.g. Social science/social care/youth work degree.
